Your package has one monthly price that includes your VitalCare plan and your Zion HealthShare membership. Every package has two prices: the standard price, paid by card, and a lower bank-draft (ACH) price. Both are shown before you choose, and you pick your payment method at checkout. A tobacco surcharge applies if you or anyone covered by your membership has used tobacco in the past 12 months ($50.00/month by bank draft).

Read the Member GuidelinesMedical cost sharing for eligible sharing requests of current or previous tobacco users who have utilized tobacco within the past 15 years are limited to $50,000 for each of the following four disease categories:
- Stroke
- Cancer -Tobacco Related
- Heart conditions
- Chronic obstructive pulmonary disease (COPD)
Although the surcharge may be removed, the sharing limitations as described in Sharing Limitations on Tobacco Use will remain.
This bundle includes a VitalCare Minimum Essential Coverage (MEC) plan and a separate Zion HealthShare medical cost-sharing membership. VitalCare MEC does not provide comprehensive major medical insurance or meet the ACA Minimum Value standard. Zion HealthShare is not insurance, and payment or sharing of medical expenses is not guaranteed. Membership is subject to Zion's Member Guidelines.
